Concrete daily report template

A concrete daily report has to answer one question months later: what went into that slab, wall or footing, when, and in what conditions. That means the generic fields plus placement volumes, ticket times, field test results and what the weather did to curing.

Who fills it out

Concrete foreman or finisher foreman, with the general contractor's superintendent reviewing. The testing agency's reports are separate but should be referenced by date and location.

Every report covers

  • Date, project, location and report author
  • Weather (morning and afternoon) and its impact on work
  • Manpower and hours by company and role
  • Work completed by area or location
  • Delays, issues and hours lost
  • Deliveries and equipment on site
  • Inspections and visitors
  • Safety notes (safety meeting held, incidents, near misses)
  • Photos with captions
  • Foreman sign-off

Concrete fields to add

FieldWhy it matters
Placement location and elementGrid lines, level and element (footing, SOG, deck, wall) tie test results and later cracks to a specific pour.
Mix design ID and supplierConfirms the approved mix was placed where the spec called for it.
Cubic yards ordered vs placedBacks up billing and shows overages or shortages on the day.
Truck tickets: batch time and discharge timeASTM C94 sets a default discharge window of 1.5 hours from batching unless the purchaser waives it. Ticket times prove compliance or flag a rejected load.
Water added on site (gallons, by whom)Water added at the truck is a common point of dispute when strengths come in low.
Field tests: slump, air content, concrete temperatureRecord the testing agency's readings or note that the agency took them.
Cylinder sets cast (count and IDs)Links break results back to the placement.
Pre-pour inspection statusRebar, embeds and sleeves signed off before placement, with inspector name.
Finishing, curing method and protectionCuring compound, blankets, wet cure or heaters, especially in hot or cold weather.
Pump or placing equipmentPump size, setup time and breakdowns explain delays.

Inspections and tests to log

  • Pre-pour rebar and embed inspection
  • Special inspection where required by the building department
  • Slump, air content and concrete temperature by the testing agency
  • Cylinder sets for strength testing
  • Floor flatness and levelness readings where specified

Weather

Log ambient temperature at placement and overnight lows. Hot weather and cold weather concreting both change curing and protection requirements (ACI 305 and ACI 306 are the common references). Rain during or right after finishing belongs in the report with photos.

Disputes this record helps settle

  • Low strength results and whether water was added on site
  • Rejected trucks and who pays for them
  • Delays waiting on inspection or pump
  • Cracking and curling, and what curing and protection were in place

Frequently asked questions

What should a concrete daily report include that a general report does not?

Placement location and element, mix design, cubic yards ordered and placed, truck ticket times, any water added on site, field test results, cylinders cast, pre-pour inspection status, and the curing and protection method.

Should the concrete foreman record test results or leave it to the testing agency?

The testing agency's report is the official record. The daily report should still note that tests were taken, by whom, and any results the agency shared on site, so the day's record is complete on its own.
